In its operational activities, the cooperative is one of the suppliers of milk in the milk supply chain system for large and small companies as well as a milk collector for breeders who are members of the cooperative. In the milk delivery supply chain activities, this cooperative has the risk of uncertainty that can cause shortages of raw materials, damage to products in collection, damage in storage, damage in delivery, even rejection due to delays, and things that can create losses for the cooperative's business. Due to the uncertainty that threatens the supply chain of cooperatives, in this study cooperatives need to assess the supply chain activities of cooperative milk delivery to companies using the SCOR method. In data processing it is divided into 3 levels, Level 1 Main Activities which have 5 criteria, level 2 has 5 criteria and level 3 has 14 criteria. These criteria will be given a score using the Snorm De Boer method per criteria and then weighted using the AHP method. The results of processing data on the performance measurement of the supply chain delivery of the Sarwa Mukti Village Cooperative Unit have a performance value of 75.69 (sufficient) and there are several things that need to be improved, such as planning expenses, arranging delivery activities, and managing assets that fully ensure smooth delivery.
